This application MUST be completed by the student.
Students must participate in the accommodation process by completing the following:
o Request their specific accommodation needs to the Office of Accessibility Resources and Services.
o Submit appropriate documentation that supports their request.
o Communicate with their Accessibility Specialist to determine if a reasonable and effective accommodation is appropriate by evaluating the nature and requirements of each class.
o Communicate to the Office of Accessibility Resources and Services any difficulties they may be having in obtaining authorized accommodations. Reasonable accommodations are authorized under appropriate circumstances and on an individual basis.
Students must otherwise be qualified to meet the academic requirements of each class, major, or program of study in which they are enrolled. Essential functions, tasks, standards, and requirements of a class, major, or program cannot be removed or waived. Possible performance standards may be applicable within the areas of observation; communication; sensory and motor function; intellectual, conceptual, integrative, and quantitative abilities; and behavioral and social requirements.
HPU must comply with the Family Educational Rights and Privacy Act (FERPA), which requires HPU to protect student educational records. More information about FERPA and HPU's responsibilities can be found on HPU's FERPA site at https://www.highpoint.edu/registrar/ferpa-guidelines/.
North Carolina General Statues require that any person having reasonable cause to believe that a disabled adult or elderly person is in need of protective services shall report such information to Adult Protective Services. North Carolina General Statues require that any person having reasonable cause to believe that a minor under the age of 18 is in need of protective services shall report such information to Child Protective Services. OARS personnel will also file a report with the Title IX/504 Coordinator: Child Abuse or Misconduct Report Form.
In accordance with Title IX, 34 CFR Part 106, and University standards, High Point University is committed to operating educational programs and activities that are free from discrimination and sexual misconduct. This mission also extends to admissions and employment. HPU takes all allegations of discrimination and sexual misconduct seriously. All OARS personnel are required to report any Title IX violations to the Title IX/504 Coordinator. This includes offenses related to sexual misconduct personally observed, as well as incidents reported to them. Reports of student misconduct should be submitted to Student Title IX/Sexual Misconduct. Reports of Employee misconduct should be submitted to: HR Title IX/Sexual Misconduct.
